Who Are Good Candidates for Mid-Face Lift Surgery?
The ideal candidates for mid-face lift surgery are female patients between the ages of 30 and 50. I do not perform mid-face lift surgery on male patients as it creates a “feminine” fullness on the face. The most common indication for mid-face lift surgery is that the aging of the lower eyelid and cheek is visually ahead of the person’s chronological age. We usually perform mid-face lift surgery when the lower eyelid is visually lengthened, the bags are very prominent, and the cheek mass is both drooping and low in volume. We can also think of mid-facelift surgery as “an extended lower eyelid surgery”. Because, patients who need mid-face lift surgery but do not have deformation in the lower eyelid are almost non-existent. The exception to this situation is young women under the age of 30. midface surgery; It is ideal for patients with structurally, developmentally and genetically weak mid-face bone structure or impaired angulation of the mid-face bone surface.

Techniques of Mid-Face Lift Surgery
Subciliary (Eyelash Root) Medium Face Lift
There are 3 basic approaches in mid-face lift surgery. The most frequently used technique among these is the “subciliary” approach, where we reach the middle face with the help of an incision made at the base of the eyelashes on the lower eyelid. The reason for this is that the lower eyelid and the midface region present both an anatomical and functional integrity.
After completing the lower eyelid surgery, an incision is made over the bone under the eye and descended under the bone membrane. Proceeding downwards under the bone membrane, all the middle soft tissue up to the rim is released over the facial bone. Then the connective tissue layers holding the midface in place are loosened and the whole tissue is lifted up as a block. When we perform the mid-face lift surgery on the lower eyelid, we detect this liberated tissue by drilling millimetric holes in the bone frame under and next to the eye. In this way, we obtain a very permanent and strong tissue fixation.
Mid-Face Lift Technique Combined Through the Temple? and the Mouth
The second technique of mid-face lift surgery is the combined temple and intraoral approach. In this approach, the lower eyelid is not used. A 2-3 cm incision is made in the scalp in the temple area and an incision of 2-3 cm is made inside the mouth to correspond to this incision. Going down the temple and upwards from the mouth, these “dissection”, surgical separation spaces, are combined with each other. Here, we hang the freed midface tissue to the outer membrane of the chewing part of the temple with long suspenders. In this technique, the face does not move completely under the eyes, but slightly upwards in a diagonal direction. The advantage of this technique is that the lower eyelid does not open at all and therefore lower eyelid complications are not observed. This technique is only suitable for younger patients.
The disadvantage is its effect is slightly weaker and cannot resolve these problems in patients with skin/muscle laxity in the lower eyelid. Since it is made orally, it can also carry the risk of infection.
Endoscopic Mid Face Lift
The third technique used in mid-face lift surgery is the “endoscopic” approach. In fact, the surgery is the same as the first 2 techniques, but the instruments used are different. In endoscopic mid-face lift surgery, some stages of the surgery are performed by seeing with a special micro camera system we call an endoscope. In this way, it becomes possible to perform the surgery through a smaller incision. By using “absorbable” fixation systems called “Endotine” in endoscopic surgery, we can reduce the need for incisions made in the mouth.

Preparation Process for Mid-Face Lift Surgery
You should inform your doctor about all the problems in your medical history while preparing for mid-facelift surgery. It is of great importance that these drugs be discontinued 1 week beforehand in patients who are taking anticoagulant drugs. Even frequently used drugs such as Apranax, vermidone, aspirin can be considered in the category of drugs that should not be used before mid-facelift surgery, as they increase the risk of bleeding. Therefore; Do not forget to inform your doctor and anesthesiologist about all the medications you have to use before mid-face lift surgery.
You should also stop the use of supplements such as ginseng, coenzyme Q, multi-vitamin tablets for a while. In addition to these, it is recommended that you do not consume products such as herbal products containing flaxseed and green tea for a certain period of time.
Smoking, which adversely affects recovery and anesthesia in mid-face lift surgery, is also on the list of things to quit at least 3 weeks before. It is recommended that patients bring a companion who can drive, as it would be inconvenient to drive after the surgery.
Mid-Face Lift Post-Surgery Process and Recovery
Mid-face lift surgery is one of the operations with the longest recovery period among facial rejuvenation surgeries. The swelling on the face after the surgery, especially in the first week, may be enough to keep the patient away from all social activities. Slanted appearance and bruises can be seen in the eyes. Within two weeks, bruising and swelling gradually decrease, but in the second week, the swelling on the cheekbones may still be at a level that may prevent the person from returning to social life. After these surgeries, patients may need to stay away from work and social life for roughly 4 to 6 weeks. Routine mouthwashes and antibiotic use are required after mid-face lift surgery performed through the mouth. Especially in oral procedures, the duration of antibiotic use may extend from 1 week to 10 days. In operations performed on the lower eyelid, there may be a slight burning and stinging sensation in the eye and eye drops are prescribed. In terms of patient comfort, patients are generally hospitalized for 1 night. The stitches on the lower eyelid and in the hair are removed within 1 week. In the first and third months, the patient is called for regular controls.
What are the Alternatives to Mid-Face Lift Surgery?
Mid-facelift surgery candidates often consider surgery because of lower eyelid-focused complaints. Therefore, lower eyelid aesthetic surgeries (lower blepharoplasty) can be an early alternative to mid-face lift surgery.
Although it is not as effective as mid-face lift surgery, removal of under-eye bags (transconjunctival lower blepharoplasty) by entering from the inner side of the lower eyelid and fat tissue transfer to the upper 1/2 part of the cheek mass is an alternative that can temporarily resolve the patient’s complaints.
Extended forms of lower eyelid surgery, although not as effective as a midface lift, can correct dimples in the lower lid/cheek transition and under-eye bags, and can tighten the superficial tissue layers of the midface.
Another alternative to mid-face lift surgery is the use of prostheses (implants). This form of treatment, which is especially suitable for male patients with weak skeletal structure, supports the skeletal structure and creates a masculine fullness in the midface area. The use of implants can be an alternative to mid-facelift surgery, or it can be used as a complement to the surgery in order to highlight the results of mid-facelift surgery.
